Think of circular shapes when you think about inclusion body myositis. Think of a clock face for insidious onset, a round skull for the atrophy involved, the circular vacuoles for what you would see on biopsy, and the sad face "irritable myopathy" on EMG. Unfortunately there are not many effective treatment options at this time - you can think about a round turtle shell because IBM is resistant to immunotherapy.
